This is a full time position.
Responsibilities
- Must participate in after hours phone rotation.
- Assist care manager with enrolled patients.
- Provides direct and indirect patient care.
- Current LPN license
- 3-5 years LPN experience preferred.
- Effective written and verbal communication
- US Citizen or Green Card holder
